All comments are reviewed
by the clinical teams at the weekly meetings and acted on. These are used to form
the base of the regular staff training sessions.

Children
with Temperature or Rash
If you are bringing a child
with a temperature or rash please advise reception and they will make arrangements
for you to wait in a separate room.
Patient
Responsibilities
Patients are expected to
behave in an acceptable manner whilst on while in the premises both towards staff and other
patients.
Patients are expected to follow the agreed care plan and take medication as directed.
Violent
Patients
Physical and verbal abuse directed
towards staff will not be accepted. The practice supports the NHS Zero Tolerance
policy and offending patients will be removed from the list without warning.
Patient Confidentiality And Data Protection
We ask you for personal information so that you can receive care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is necessary that medical information about you is shared between members of the practice team.
Much is said in the press about patient data being held on national databases. In fact, there has been a national database in existence for more than 50 years! Originally held on paper it has since the 1960s been increasingly computerised in various ways. In the past this system held only 'demographic' data (eg patient's name, address, and date of birth). Access was slow but the system could be used to confirm patients GP and entitlement to NHS services.
Today, electronic transfer of medical records between practices is possible. We at Streatfield Health Centre are able to both send and receive data, depending on the development of the distant surgery.
Hospitals can and do access the national database to confirm you are registered with a GP if you attend A&E. There has been much debate about this access increasing to the extent that hospitals can also access your medical records when you attend, in order to see current medication and allergies.

Freedom Of Information – Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available from reception.
